Does your 6th-12th grade student athlete need a sports physical?
Schine Health will be at Centralia High School on Monday, August 18th from 10 am-1:30 pm offering FREE sports physicals. Enter throught the main doors.
Please be advised: Parent attendance is STRONGLY ENCOURAGED. Please arrive with the completed forms SIGNED by a PARENT/LEGAL GUARDIAN, to be granted a sports physical.
There will be forms available to fill out on site! Please use the link below to print the required forms. If you have an insurance card, please bring it with you, but individuals will not be billed.
